Pre-Authorized Donation Form

The attached form will permit the parish to make automatic withdrawals from your bank account, or to charge your credit card. For bank withdrawal, please include a voided cheque with the form. For reasons of security, we recommend that the form - and voided cheque if a bank account withdrawal - be returned in the collection basket, by Canada Post, or by delivery to the Parish Office.
